Shrine Parish of Our Lady of Fatima is a Roman Catholic and Marian church located in Calubian, Leyte, Philippines.

History
Its former name was Santander and then renamed Eulalia after a woman renown for her leadership. The town was carved out from the municipality of Leyte through Executive Order No. 4 issued by Governor General Charles Yeater. The church was established in 1954.
Architecture
The church is rectangular in shape with a dome rising above the altar. Its three-cornered bell tower is in the epistle side in the front yard. The facade has an overhang to provide shade on the entrance and its wall is pierced with rectilinear slit windows.
Celebration
The feast day of Our Lady of Fatima is August 16.
